CyberKnife: The “Life-Extending Miracle” for Brain Metastases from Lung Cancer

This title was given to me by the family member of a patient with brain metastases from lung cancer. She carefully documented the patient's entire medical journey—from the diagnosis of lung cancer in 2011 to the six courses of CyberKnife treatments I performed by May 2025—and concluded that “CyberKnife is a life-extending miracle for patients with brain metastases from lung cancer”.

2011, February – At the Chest Hospital, the patient underwent right upper lobe resection for lung cancer, followed by four cycles of chemotherapy with the cisplatin–pemetrexed (GP) regimen at the same hospital.

Discharge summary: Bronchogenic carcinoma, moderately to poorly differentiated, p-T2aN1M0.

2012, September – At the same hospital, the patient received a minimally invasive surgery for a left lung lesion.

Pathology: Invasive adenocarcinoma, lepidic subtype, moderately differentiated.

Genetic testing: No EGFR mutation.

Postoperative treatment: 3 cycles of chemotherapy.

2013, February – At Huashan Hospital Pudong, CyberKnife treatment was performed for metastatic lesions in the cerebellum and right frontal lobe.

2013, December – Gamma Knife treatment for a left frontal lobe metastasis at Gamma Knife Center in Huashan Hospital.

2014, January – Four cycles of temozolomide chemotherapy.

From 2011 to 2020, the patient continued traditional Chinese medicine (TCM) treatment at Shuguang Hospital and Longhua Hospital.

2020, September – Recurrence in the right cerebellum and left frontal lobe. Treated again with CyberKnife.

Subsequent genetic testing revealed a KRAS G12C mutation.

2021, May – Surgical removal of a right adrenal gland tumor, with partial adrenal preservation.

2022, May and July – Two courses of CyberKnife therapy targeting right adrenal and posterior abdominal lesions.

2022, September – 2023, March – Traditional Chinese medicine treatment in Haikou.

2023, April – Joined a clinical trial at the Cancer Hospital using JAB-21822 combined with JAB-3312 (Jacobio Pharmaceuticals); withdrew from the trial in November 2023.

2023, November – 2024, February – Four cycles of temozolomide at the Cancer Hospital.

2024, March – Began four cycles of combined immunotherapy and chemotherapy (tislelizumab + carboplatin + pemetrexed).

2024, June 11–28 – Radiotherapy (14 sessions) for a new lesion in the right lung at the same hospital.

2024, July – Began traditional Chinese medicine treatment at Shanghai Yueyang Integrated Traditional Chinese Medicine and Western Medicine Hospital under Dr. Xu Ling.

2024, August 2 – Received tislelizumab immunotherapy at the Cancer Hospital (only one session). Later developed gait instability and was admitted to Yueyang Hospital, where hypothyroidism was diagnosed. Began levothyroxine (Euthyrox) therapy and discontinued immunotherapy.

2024, August – Radiofrequency ablation for the left lung lesion at Huashan Hospital North.

2024, September – CyberKnife treatment for a right cerebellar metastasis at Huashan Hospital Pudong.

2025, February – Began Garsorasib at the recommendation of the Cancer Hospital, from February 5 to April 29.

CT at the end of April showed the largest lung lesion had increased from 4 cm to 5.1 cm compared with January. After discussion, the family decided to forgo paclitaxel chemotherapy and instead proceed with CyberKnife treatment.

2025, May 9 – PET-CT at Huashan Hospital Pudong:

No bone metastases; right lung lesion enlarged to 6 cm with high metabolic activity; right cerebellar and frontal lobe edema; multiple nodules in the left lung, the largest measuring over 2 cm, exhibiting malignant features such as lobulation and spiculation.

2025, May 12 – At Professor Wang Enmin's special outpatient clinic, CyberKnife treatment was recommended.

2025, May 15 (afternoon) – The patient was admitted for CyberKnife treatment of the right central-type lung cancer.

The doctor became good friends with the patient and their family. The treatment plan involves two staged CyberKnife courses for the right central-type lung cancer.
